How to Use the Mounjaro Pen: A Step-by-Step Guide for Beginners
Starting a new injectable medication can feel daunting at first, but it doesn’t have to be. At Fermanagh Clinics, we help patients across Northern Ireland get started on GLP-1 treatments like Mounjaro with confidence, support, and the right guidance. If you’ve recently been prescribed Mounjaro for weight loss or type 2 diabetes management, this simple guide will show you how to use the Mounjaro pen safely and effectively.
Whether you’re using it for the first time or just want to make sure you’re doing it correctly, we’ll walk you through preparation, injection technique, and aftercare.
What Is the Mounjaro Pen?
Mounjaro, or tirzepatide, is a weekly injection that mimics the effects of natural hormones to regulate blood sugar and support weight loss. It comes in a pre-filled, single-use pen that’s designed to be self-administered. No priming, mixing or assembly is required.
Each pen contains one dose and is colour-coded according to strength. The pen is injected subcutaneously (under the skin) into the abdomen, thigh or upper arm.
Before You Start
Before your first injection, you’ll need to:
-
–> Speak to a clinician to ensure Mounjaro is appropriate for your medical condition and weight management goals
-
–> Read the full patient leaflet provided with the medication
-
–> Wash your hands and gather your supplies: the pen, an alcohol wipe (optional), and a sharps bin for disposal
If you’re unsure about any step, one of our clinicians at Fermanagh Clinics will be happy to show you how it works during a face-to-face consultation.

Step 1: Choose the Injection Site
Mounjaro should be injected into a fatty area just under the skin. The most common sites are:
-
–> The front of your thighs
-
–> The lower abdomen (at least two inches away from the belly button)
-
–> The back of the upper arms (this can be harder to reach on your own)
Make sure the injection site is clean and dry. Avoid areas with bruises, scars, or hard lumps. Rotate injection sites each week to avoid skin irritation.
Step 2: Inspect the Pen
Check the pen’s label to ensure it’s the correct strength and hasn’t passed the expiry date. The solution inside should be clear and colourless. If it’s cloudy or contains particles, do not use it.
Let the pen come to room temperature if it’s been in the fridge. This can make the injection more comfortable.
Step 3: Remove the Cap
Hold the pen with the grey base cap pointing down. Pull off the grey base cap in a straight motion. Do not twist it.
Once removed, the pen is activated and should be used immediately.
Step 4: Place the Pen on the Skin
Hold the pen straight against the chosen injection site so that the base is flat against your skin. Press firmly and ensure full contact is made.
Do not press the button until the pen is in position.
Step 5: Press the Injection Button
With the pen pressed firmly against your skin, press the purple injection button at the top. You will hear a loud click, which means the injection has started.
Continue holding the pen in place. You will hear a second click within about 10 seconds — this indicates that the injection is complete.
Wait an additional few seconds before removing the pen from your skin to ensure the full dose has been delivered.
Step 6: Dispose of the Pen
After use, the Mounjaro pen should be placed in a sharps disposal bin. Do not throw it in your general waste. You can return full sharps bins to the pharmacy or your healthcare provider for safe disposal.
Step 7: Store the Next Dose Properly
Unused pens should be stored in the fridge between 2°C and 8°C, but not frozen. You can also keep a pen at room temperature (below 30°C) for up to 21 days. Keep the pens in their original packaging to protect them from light.

Tips for a Comfortable Experience
-
–> Inject on the same day each week to maintain routine
-
–> Apply gentle pressure (not rubbing) to the site after injecting if there’s any soreness
-
–> Stay well hydrated and eat a balanced diet to support the medication’s effects
-
–> Track your doses and side effects using a journal or app
-
–> Contact a healthcare professional if you experience any severe side effects or signs of an allergic reaction
